THE LIQUID FACELIFT: MYTH VERSUS REALITY FOR OLDER WOMEN


The widespread use of Botox and fillers has revolutionized our industry and in combination has been coined by the press as a "Liquid Facelift".  Indeed, the use of fillers to restore volume to the temples, cheeks, nasal labial folds, marionette lines and lips gives incredible results with minimal risk, scars or downtime.  This approach works well for women in their thirties and forties but by the time a women reaches her fifties she often needs more than simply a little plumping here and paralysis there.  

When a woman needs facial rejuvenation, especially of the lower face, there is no substitute for a properly performed face and neck lift.  With the new short scar facelift (different from the “Lifestyle Facelift” that accomplishes very little) there is minimal risk with minimal scarring yet the results are so striking that patients usually feel they look ten to fifteen years younger. 

The most common question I get about face lifts is "how long will my results last"?  The answer is a face and neck lift lasts FOREVER, but since gravity and unfortunately the aging clock doesn't stop for anyone, you will continue to get older...fortunately you will always look ten to fifteen years younger than you would have looked had you not done the surgery! 

How long fillers and Botox last is simple...Fillers are usually reinjected every six months to a year while Botox must be done three times each year to maintain its effect. 


The best results are always achieved with a combination of procedures.  Once you've had your face and neck lift, you should still continue to use Botox, fillers and skin rejuvenation procedures (chemical peels, laser treatments and dermabrasion) to help you maintain the optimal results you're looking for...you just may not need as much after a face and neck lift.

PERSONALIZED BOTOX...”HAVE IT YOUR WAY”

Botox has become one of the most sought after cosmetic procedures in the US due to its ease, obvious quick gratification, and relative low cost.  My approach to Botox is slightly different in that I personalize treatments to provide the most natural results possible.  I want to avoid at all costs that overdone look or even full facial paralysis that hinders communication in daily life.

When a patient first starts Personalized Botox, a conservative treatment is performed to soften the wrinkles that are of concern.  Usually, the dosage of an initial Botox treatment is sufficient to achieve the desired results, however, if two weeks pass and further improvement is desired the patient returns for a second session.  After the initial treatment(s), a patient’s Personalized Botox dosage (quantity) is then established, and follow up visits are usually unnecessary.  

My goal is simply to soften ones appearance, not to erase all of their character.  When initiated early enough and performed two to three times per year, the results are astounding and allow patients to age gracefully without the tell tale signs of wrinkles. 

There are cost benefits of Personalized Botox too.  You only pay for what you use and need, unlike other plastic surgeons who charge a flat fee for every patient regardless of the amount of Botox used.

LIMB LENGTHENING FOR FUN??? Really??!!

Dr. Eric Sadeh was interviewed tonight by TV Talk Show Host, Dorothy Cascerceri on Celebrity Corner.  Dorothy asked about the Limb and Leg Lengthening.  Here's what Dr. Sadeh thinks about it.....

Originally developed by Dr. Ilizarov in the 1960’s, limb lengthening was intended for people with dwarfism and children with one leg significantly shorter than the other.  Limb lengthening is now finding its way into the world of cosmetic surgery -hmm...is there too much pressure to look like a model (Gisele pictured below)?  On average, patients can gain an extra 2 to 3 inches...IS THIS REALLY WORTH IT???

Limb lengthening involves breaking the bones of your leg and slowly “distracting” them apart allowing new bone to form and nerves, arteries and veins to stretch.  Not only is the procedure extremely painful and takes months, but there are a number of serious consequences that have to be avoided.  The procedures should be performed in the most sterile of environments, ideally a laminar flow operating room in a hospital to prevent infections and ultimate failure of the procedure.  The metal distraction devices can break or malfunction, necessitating further surgery to remove and replace them.  The incision sites and soft tissue injuries from the distraction device may leave significant scarring.  Nerve damage can be permanent and lead to functional issues with the legs and chronic pain.

TOE-BESITY - Should you consider this surgery?

Dr. Eric Sadeh was interviewed tonight by TV Talk Show Host, Dorothy Cascerceri, on Celebrity Corner. Dorothy asked about the latest craze "Toe-besity" procedures.  Here's what Dr. Sadeh thinks about the Toe-besity.....

Fat toes can be defatted or thinned by excising wedges from the soft tissue...unsightly long toes can be shortened by excising a segment of bone and foreshortening the toe...etc. 

While none of us can argue that seeing women stuff their feet into all sorts of closed and open toe shoes can be painful, does this really justify cosmetic surgery to correct these situations?

While these procedures will undoubtedly be performed primarily by Orthopedists and Podiatrists, I believe they should only be reserved for the most severe and functionally compromising conditions. 


Our feet are essential for standing and locomotion, and due to their dependent position on our bodies, they tend to swell and ache during the course of the day.  Surgical incisions heal more poorly on the foot and any residual scaring tends to be more painful than on other parts of the body.  Combine this with the fact that your toes are essential for “pushing off” and propelling your body forward.  Imagine the discomfort you would feel with poorly healed, chronically painful scars when the weight of your body is applied.  For the most part, aesthetic surgery for the toes should be avoided, but if you insist, find a surgeon that has sufficient background in treating other ailments of the feet before you sign up. 

VAMPIRE FACELIFT

Dr. Eric Sadeh was interviewed tonight by TV Talk Show Host, Dorothy Cascerceri on Celebrity Corner.  Dorothy asked about the Vampire Facelift.  Here's what Dr. Sadeh thinks about the Vampire Facelift.....

Catchy name for a Vampire obsessed society...but does it really work??? 

Made popular by Kim Kardashian, the Vampire Facelift involves taking your own blood, separating out the red and white blood cells from the plasma, activating the components, and then re-injecting the concentrated solution containing a platelet rich-fibrin matrix into the face.  Initially, this solution has the effect that any filler would have when injected into the face, however, that fullness is short-lived.  The real long term benefit is that it could stimulate angiogenesis (new blood vessel growth into the injected area) and increased collagen synthesis.  The result would be thicker, healthier skin that could last for several years. 

The problem is that to date there are no scientific studies showing that the Vampire Facelift really works.  Even plastic surgeons that tout its benefits recommend that it be repeated one to three times to see maximal effects.  They also suggest that the well known Hyaluronic acid fillers such as Juvedarm and Restylane be used in conjunction to enhance the effects of the Vampire Facelift...makes sense, because the procedure does not add any lasting volume to the face, it just helps improve the quality of the skin.
